Parents can register students at Brookline High School, 115 Greenough Street, Brookline, Massachusetts. Registration is by appointment only. Please call Linda Wentzell, Registrar at 617-713-5006.

Do not mail registration forms to schools. All registrations must be done in person.
The following documents are necessary to register:
*Proof of Residency
-A tax bill or deed is required if you own your home/condo along with a current utility bill.
-A lease is required if you are renting, dated within the year, along with receipt for first payment and a current utility bill.
-A notarized letter from the owner if you are residing with them, along with the owner's tax bill.
*Original Birth Certificate (in English)
Passport of students not born in United States.
*School Records (in English) Complete records are required for class placement.
Official, signed, high school transcripts are required for transferring students. (Grades 9 through 12)
Students registering for Grade 9 are required to bring copies of their 7th and 8th grade report cards that show promotion.
*Immunization Record (in English)
You will meet with the school nurse or secretary in the clinic. A physical examination within the last year should be presented and immunizations will be reviewed. (Immunizations required by the Commonwealth of Massachusetts are listed in the Registration Packet.)
Approval by the school clinic is required prior to class scheduling.
A physical exam must be obtained in the United States.
