    PSB CORI Application Form  
     
    CORI forms are required for all volunteers in Pierce School. This includes field trips, math league, school play volunteers, event volunteers, etc.  If you volunteer now or plan to volunteer in any capacity, it is important that you have a current CORI form on file.  CORI forms are valid for 3 years only. The form can be downloaded below and must be accompanied by a photo ID when you bring the completed form to the Main Office.
