What do I need for the registration process?

You must gather the following documents before you can begin the registration application. This ensures that school staff has access to developing your student(s) individual learning profiles and supports so that they have a smooth transition into their first day of school.

- Parent/Guardian ID
- Valid US driver’s license/ID card, Passport, Military ID, or Other Government Issued photo ID
- Valid US driver’s license/ID card, Passport, Military ID, or Other Government Issued photo ID
- One (1) Proof of Residency: Temporary residence in Brookline (4 weeks or less), or for the sole purpose of attending the public schools, will not be considered as residency.
- Most recent mortgage payment
- Most recent property tax bill
- HUD-1 Settlement + Payment, OR
- Active Leasing Agreement
- Two (2) Proofs of Occupancy: Utility statements, bills, and/or work orders dated within 45 days of your registration appointment. Proofs of occupancy must show that service has been established at the residential address under the parent/guardian's name. Acceptable proofs include:
- Cable/Internet Bill, Statement of Service, or Work Order
- Electric Bill, Statement of Service, or Work Order
- Gas Bill, Statement of Service, or Work Order
- Oil Bill, Statement of Service, or Work Order
- Home Telephone Bill, Statement of Service, or Work Order
- Home or Renter's Insurance Policy
- Documents About your Student(s): These documents must be accompanied by authorized translations if the original version is not in English.
- Birth Certificate or Birth Record
- Immunization Records and most recent Physical Examination
- Progress Reports or Transcripts (dependent on the student's grade level)
The records required for registration depend on the grade that your student is entering. Each grade level and the corresponding required records are found below.
- Grades 1-8: Most recent report card and/or progress report
- Grade 9: Grade 7 and Grade 8 official transcripts
- Grade 10: Grade 8 and Grade 9 official transcripts
- Grade 11: Grade 9 and Grade 10 official transcripts
- Grade 12: Grade 9, Grade 10, and Grade 11 official transcripts
- Other Important Documents Relevant to the Student's Well-Being, if applicable:
- Active Custody Documents
- Active Court Records
- Other Academic Records or Supports, such as IEPs, 504s, and/or Early Intervention Services
Please note that these documents will be necessary for when you complete the online registration. Original copies of these documents must also be presented during your in-person registration appointment with the Office of Registration and Enrollment.
Failure to provide these documents at the registration appointment may delay your student(s) enrollment. A student cannot begin school until all of the required documentation has been received.

Residency Certification Agreement
A Residency Certification Agreement may be required for established/new residents whose residency falls under the following categories:
- Tenant-At-Will
- Sublet Agreement
- Living with Family/Friends
- Live-in Employment (e.g. Nanny, caretaker, etc.)
- Rental of rooms in a Brookline residence- Student over age 18 living with family/friendsOther situations may also require a Residency Certification Agreement upon request by the Office of Registration and Enrollment.
